Case Overview
This 45-year-old patient traveled from Indonesia to see Dr. Chia Chi Kao after undergoing a tummy tuck in Jakarta approximately two years earlier. She was left with a high, thick scar, a persistent abdominal bulge, fullness in the mons pubis, and visible distortion of the abdominal contour.
At first, the case appeared to be a revision of a mini tummy tuck with an epigastric hernia. During surgery, however, Dr. Kao discovered that the patient had actually undergone a full tummy tuck and had developed a huge, thick seroma capsule. This made the operation far more difficult than expected.
What Went Wrong With the First Tummy Tuck
Dr. Kao often sees tummy tuck results where the scar is too high, too straight, too thick, or placed under too much tension. In some cases, the mons pubis is pulled upward, the lower abdomen looks distorted, and the waistline is not properly sculpted.
In this patient’s case, the distance between the belly button and the scar was extremely short, making revision especially challenging. There was very little available lower abdominal skin to work with, and Dr. Kao had to carefully recruit more skin from above in order to lower and improve the scar line.
The Unexpected Seroma Capsule
During surgery, Dr. Kao encountered extensive internal scar tissue and a large seroma capsule. A seroma is a collection of fluid that can develop after surgery. When it persists, the body can form a thick capsule around the fluid cavity.
In this case, the seroma capsule was thick, firm, and difficult to remove. Dr. Kao described the internal scar tissue as extremely challenging, with tissue that felt almost like cement. The entire capsule had to be removed before the abdomen could be properly flattened and reconstructed.
Dr. Kao’s Revision Plan
Dr. Kao performed a complex redo tummy tuck with 360-degree waistline sculpting. The goal was not only to slim and contour the waist, but also to recruit additional skin so the old scars could be removed and the new scar could be placed lower.
The revision plan included extensive liposuction and contouring of the waist, abdomen, flanks, and back. Dr. Kao also revised the abdominal scar, addressed the mons fullness, removed the thick seroma capsule, flattened the abdominal bulge, and created a smaller, more natural-looking belly button.
Despite the difficulty of the case, Dr. Kao was able to excise the bad scars, create a pencil-thin scar below the panty line, improve the abdominal contour, and give the patient a smaller waistline.
10-Month Post-Op Result
At 10 months after surgery, the patient returned with a dramatically improved abdominal contour. The previous bulge was corrected, the waistline looked more refined, and the scar was faint and low enough to be hidden below the panty line.
Dr. Kao also improved the shape of the belly button and contoured the back and waistline to create a more balanced figure. The patient was very happy with her result, and her husband was described as speechless.
